The following process is to be taken for employees applying for a flexwork agreement:

  1. Read and understand the UCSB flexwork guidelines and agreement

  2. Take the Work Suitability Assessment, and consider the results when completing the duties and assignments section of the agreement. A discussion between you and your supervisor/manager may be required to finalize the tasks and performance measurements best suited for your job.

  3. Complete the agreement as best you can, sign and submit your application to your immediate supervisor/manager. The application includes the agreement, Work Suitability Assessment results, and ergonomic and home office safety agreement, if applicable. Portions of the agreement will still require input from your supervisor/manager. If requesting a telework agreement, you will also need to read the ergonomic and home office safety agreement.   4. Set aside time to meet with your supervisor/manager to review the application and to discuss the terms of your participation in the program. Your supervisor/manager may approve the proposed flexwork arrangement. Your supervisor/manager may also recommend an alternate flexwork program. You and your supervisor/manager must agree on a mutually agreeable and appropriate flexwork arrangement, as it is the responsibility of the employee to make the case for a flex work arrangement.

  5. When a telework program is approved, you may choose to take the Employee Suitability Assessment, a developmental tool designed to provide guidance and support around telework skills. (The results are not to be used for performance evaluations, or for denying a flexible schedule arrangement.) The Employee Suitability Assessment will be emailed to you with directions. Once complete, email the assessment answer sheet to the address as directed and you will receive a completed assessment for you to use with your supervisor/manager. This assessment tool is optional but is recommended for employees new to telework.

  6. Once all steps are taken, your supervisor/manager must submit the completed and signed agreement to the department head/designee for final approval.

  7. If the employee wishes to renew this agreement, he/she will formally request a renewal in writing. Renewal of the agreement is subject to review and approval by the employee’s supervisor/manager. Participation in a flex schedule program may be terminated at any time upon the request of the supervisor/manager, upon the request of the employee and agreement of the supervisor/manager, or with a two week notice if that is acceptable to the supervisor/manager.
